Summary:
The RN Care Resource Coordinator functions as an advocate for the patient and assists patients to reach optimal function and health in an environment that will support their needs. The RN Care Resource Coordinator applies the nursing process of assessment, planning, implementation, monitoring, evaluation, and coordination of care to meet the patients' health care needs in collaboration with the primary care team to ensure safe, quality, and cost-effective care.
Job Qualifications:
- Bachelor's degree in nursing is required; Master's degree in nursing is preferred.
- Must have a Georgia Registered Professional Nurse license.
- Three or more years of experience in clinical practice in an ambulatory setting or equivalent combination of education and experience in a hospital inpatient, outpatient, behavioral health, or social service environment is required.
- Experience in motivational interviewing/health coaching/goal setting is required.
- Certification in Care Coordination or willingness to complete within 1 year is required.
- Basic Life Support (BLS) from American Heart Association is required.
