Summary: Serves as facilitator for pharmaceutical care team; participates in ongoing education of team members; coordinates quality improvement initiatives of team; ensures the safe, efficacious, and cost-effective use of medications in assigned patient populations.
Minimum Qualifications:
- Formal Education Required:
a. Doctor of Pharmacy
b. Licensed or license eligible by the Ohio State Board of Pharmacy
- Experience & Training Required:
a. Completion of residency training in pharmacy practice or three (3) years of increasingly responsible experience in a pharmaceutical care position which has provided skills in pharmacy education and quality improvement.
b. Experience in Oncology Practice (Certification preferred)
- Other Skills, Competencies and Qualifications:
a. Ability to effectively communicate verbally and in writing
b. Ability to operate a computer terminal, hand-held calculator, and other office equipment; manipulate fine instruments; and prepare pharmaceutical entities in minute proportions.
c. Ability to work well within a team environment: by accepting and offering honest and constructive feedback; by supporting team goals and encouraging other team members; and by collaborating with patient care team members to consolidate work, solve problems, create new methods, and/or reduce costs.
d. Ability to be self-directed in executing responsibilities within a team structure.
e. Working knowledge of Ohio pharmacy law.
f. Flexibility/adaptability to meet environmental constraints and demands common with changing reimbursement and managed care targets.
g. Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity.
- Level of Physical Demands:
a. Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently.
b. Hazards: Exposure to blood, blood fluids, and sharps; risk of falls, strains, contagious diseases, cuts from broken glass, needle puncture, etc. Possible exposure to selected hazardous substances (e.g., alcohol, chemotherapeutic agents, etc.)
