SummaryThe Triage Nurse (Hospice) is an integral part of the Hospice interdisciplinary team and is responsible for the excellent delivery of care through triage calls after hours and on Holidays.
Responsibilities- Receive calls from patients and/or family members, respond appropriately, and assess problems focusing on the delivery of high-quality, patient-focused, compassionate care.
- Timely and accurate documentation of calls received within the electronic medical records system, including the processing of workflow associated with the clinical record.
- Knowledge of end-of-life symptom management and pain control to effectively address the needs of patients in a crisis situation.
- Coordinate with the agency on-call nurses to deliver high-quality nursing care and schedule nursing assessments as required in a timely manner to meet the needs of the patients and families.
- Maintain effective communication with patients/families, employees, physicians, vendors, and potential referral and payer sources during after-hours on call.
- Update all hospice staff and members of the interdisciplinary team regarding patient changes, vendor and/or facility changes to ensure compliance with the patient's plan of care.
- Assist in overseeing the utilization and ordering of medications, DME, and supplies related to the plan of care to support the organization’s financial performance.
- Communicate appropriately with the medical director, attending physicians, etc., regarding patient status changes or needs.
- Ensure appropriate education regarding all updates/processes in the electronic medical record, relative state and federal regulations, documentation processes and needs, etc., by attending mandatory educational offerings and in-services.
- Facilitate orientation of new personnel as assigned.
- Attend and actively participate in weekly Interdisciplinary team meetings as assigned.
- Knowledge of and adherence to all policy and procedures.
- Maintain compliance by documenting accurately and timely within the organization's electronic medical record.
- Other duties as assigned.
Education and Experience- Must be a licensed Registered Nurse in good standing with no restrictions in all states of practice. Compact licensure preferred.
- Must have two (2) years' experience as a Registered Nurse with at least one year of direct patient care in a hospice, home health, or oncology setting.
