Program Summary:
Center For Family Services is seeking experienced and professional Case Manager for the Crisis Diversion Home Program. This is a 24-hour residential group home for adults who recently experienced a mental health crisis. The Crisis Diversion Home provides a nurturing, supportive, and structured environment where our clients can develop trust, learn to manage their illness and live independently.

Duties and Responsibilities:
- Provide full range of case management services to CDH clients as follows: Referrals and linkage and advocacy to housing, financial, medical, educational, vocational, legal, spiritual and other needed/indicated community resources and services in support of client recovery and community integration. Case service coordination on behalf of consumer with families and other behavioral, medical, housing, hospital and screening service providers to ensure holistic management of clients' total health, treatment and recovery needs and goals after program discharge.
- Provide (using program vehicle) and/or arrange for transportation services for clients to afford access to resources and services described in #1 above.
- Co-facilitate IMR, psychoeducational, WRAP planning and other therapeutic groups.
- Complete documentation and reports to be submitted to supervisor on a timely basis.
- Meet with supervisor on a regular basis and participate in regularly scheduled program meetings.
- Take responsibility for professional development through attendance at relevant workshops and seminars.
- Participate in Center and Program performance improvement activities.
REQUIREMENTS:
- Bachelor's degree (from an accredited University) in Social Work, Psychology, Counseling, or a related behavioral health field.
- 2 years supervised experienced in working with mentally ill and/or substance abuse clients.
- Current driver's license with a good driving record of at least three years.
