Program Manager ACO & Population Health
About the Job:
The Program Manager for Accountable Care and Population Health reports to the Director of Value-Based Care in support of the SNHH ACO Executive and respective committee structure. The Program Manager helps in the development and implementation of a cohesive approach to measuring and promoting health equity, access, quality, cost, utilization, and patient experience for payment and care delivery innovation initiatives including the ACO. As part of the Value Based Care team, the Program Manager for the ACO provides strategic, operational, and analytical support for ACO and Value Programs. The role is key to the development, implementation, and sustainability of initiatives and programs to support the ACO and population health. This includes performing health care service delivery research, and regulatory/policy analyses that are used to identify, evaluate, and develop recommendations to support ACO planning and execution. In conjunction with SNHH leadership and teams, the Program Manager is responsible for the development, testing, evaluation, and implementation of projects associated with the ACO strategic plan and programmatic deliverables in other downside risk arrangement contracts.
What You’ll Do:
- Provides program management to the following: the care gap program, promoting interoperability, ACO performance monitoring and reporting and ambulatory quality programs.
- Manages accountable care and quality programs and initiatives, medium to large scale, with significant complexity within the ACO, health equity, and population health purview.
- Coordinates cross-functional population health infrastructure and programming.
- Maintains the health equity road map and oversees the management of the initiatives.
- Oversees any associated claims software contracts, and training, optimization, and translation into operations. Performs literature reviews and appraisal of evidence.
- Develops and implements strategy for quality data supplementation for value-based care programs.
Requirements:
- Bachelor's degree in Business or Healthcare required. Master's degree in Healthcare/Public Administration, Public Health, or related field preferred.
- CPHQ, Strategy, or Project Management certification preferred.
- Required experience: Demonstrated knowledge of health care administration and service delivery. Demonstrated understanding of population health strategies. Foundational understanding of health care informatics (i.e. PMPMs, Utilization rates, risk adjustment, evidence-based or quality metrics) required. Understanding of health care quality metrics and performance.
