MHC Healthcare is seeking a bilingual Nurse Practitioner with at least 1 year of recent Primary Care or Family Practice experience to join our Integrated Care Team at the MHC West Side Health Center, located in Tucson, AZ. MHC Healthcare is a Federally Qualified Community Health Center (FQHC), with 17 sites in Tucson and Pima County. The Family Nurse Practitioner promotes and maintains patients' health by providing medical services to the extent to which they are licensed and/or certified. The Nurse Practitioner uses independent judgment in carrying out daily job responsibilities. The position will be supervised by the Medical Director. The ideal candidate will have the following education and experience:
- Master’s degree (MSN) from an accredited school of nursing, combined with at least 1 year of recent Primary Care or Family Practice experience.
- Candidates without the MSN, but whose Nurse Practitioner certification is current and was grandfathered-in prior to the Master’s degree becoming a requirement for the credential, will be considered if they also have the required hands-on experience.
Duties and Responsibilities of the Nurse Practitioner – Family Practice include, but are not limited to:
- Under the general supervision of the Medical Director, plans, directs, and evaluates patients in a primary care setting.
- Identifies short-term and long-range patient care issues that must be addressed; providing information and commentary pertinent to deliberations; recommends options and courses of action.
- Refers patients to physicians of specialty as needed, with proper documentation needed to continue care for patients.
- Collaborates with other physicians in the care of patients as needed.
- Provides continuity of care by developing and implementing patient management plans.
- Diagnoses and treats common acute health problems.
- Diagnoses, treats, and monitors chronic health problems.
- Provides health maintenance care such as physical examinations for all ages, well baby checks, and gynecological care.
- Collaborates with physicians as needed on complex, unusual, and high-risk cases.
- Orders and interprets diagnostic tests and prescribes proper medications. Follows up with patients through telephone and mail correspondence. Documents patient contacts in patient charts.
- Promotes wellness and self-care of patients and their families by providing health education including counseling on exercise, nutrition, well baby care, family planning, and diabetic issues.
- Performs outpatient procedures such as suture removal and other procedures within the scope of practice.
- Attends continuing education conferences as required to maintain licensure.
- Promotes community awareness on health care issues.
- Promotes and participates in off-site activities such as Care-Fairs, School-Based Programs, and Marana Treatment Center as requested by the Medical Director.
- Ensures proper documentation on patient charting, patient encounter forms, insurance forms, and any other additional information pertaining to patient care.
- Participates in other assigned duties as designated by the Medical Director and/or Chief Executive Officer.
