Job Title: Nurse Clinician - Tracheostomy/Home Ventilator Program
Work Schedule: Monday-Friday Days
What you will do:
The role functions with demonstrated expertise in the delivery of direct patient care to children in the Tracheostomy Home Ventilator Program and their families. The role requires ongoing collaboration with multiple providers involved in the care of these patients and patient populations. The Clinician, with advanced clinical knowledge and skill, partners with the Tracheostomy/Home Ventilator Advanced Practice Nurse and Interdisciplinary team to assess needs, plan care, and provide nursing care to individuals and groups. The Clinician is a recognized expert resource and consultant to healthcare providers, within Children’s Wisconsin and the community at large.
Main Responsibilities:
- Assessment and Decision Making: Applies principles of growth and development in all stages of the nursing process; communicates in a manner most appropriate for child and family; makes independent nursing decisions and integrates medical plan of care into care pathway for patient and family. Participates in decisions about procedural changes, protocols, or equipment modifications. Negotiates systems to implement change across systems.
- Case Management: Identifies need for case management and initiates appropriate referrals in both inpatient and ambulatory settings. Collaborates with interdisciplinary colleagues to implement case management plans. May provide formal case management to select patients.
- Home Care Management: Assesses needs of patient/family for care at home; individualizes plans for care at home; identifies and coordinates necessary and available resources to accomplish home care needs.
- Teaching: Understands/applies basic learning principles to both children and adults; is aware of and utilizes available resources. Serves as resource to inpatient nursing staff in development and implementation of teaching plan.
- Collaboration/Advocacy: Effectively communicates with caregivers and providers on patient's needs and progress relative to established outcomes. Coordinates interdisciplinary team for children/families in Trach/Home Vent Program.
- Research and Quality Improvement: Applies current research findings into practice, participates in data collection and population-based projects; disseminates and incorporates current research into clinical practice.
- Community Outreach and Program Development: Assesses community needs related to complex care of this patient population, participates in and evaluates community education and government advocacy needs for this patient population.
- Standards of Care: Assists in setting expected standards of care for this patient population.
What you will need:
- RN State License
- Current CPR certification: Basic Life Support (BLS) for healthcare providers through American Heart or American Red Cross
- Bachelor’s degree in Nursing
- Three to four years pediatric and specialty critical care/neonatal nursing experience with emphasis on managing patients with chronic and complex health care needs
