The LPN oversees Medication Administration, monitors client health, and collaborates with program staff around client treatment. The hourly rate for this role is $24.00/hr - $39.00/hr.
RESPONSIBILITIES :
- Facilitate the monitoring of the client's self-administration of medication.
- Appropriately document all medication taken by residents.
- Monitor clients for any adverse reactions to medications and take appropriate actions, and report as necessary.
- Accept and document deliveries of medication.
- Perform daily medication reconciliations to verify counts of all clients' medication.
- Comply with all medication policies and procedures.
- Secure, monitor, and distribute controlled medications, in compliance with all agency, city, state, and DEA regulations.
- Coordinate with other nurses and staff to coordinate patient care and medication.
- Document and share all non-compliance with medication regimens.
- Appropriately discard medication as required, do so and document in accordance with all policies and regulations.
- Protect client confidentiality except in cases to coordinate care with staff
- Conduct groups with residents the promote medication education and medication compliance.
- Coordinate processes to improve/evaluate medication administration. Makes recommendations.
- Assist with referring clients to appropriate healthcare providers/specialists.
- Coordinates with the client's primary care physicians to inform decisions regarding the management and treatment of problems identified and provides care as prescribed by a physician.
- Assist with the management of client prescriptions with medical providers and various pharmacies, with a focus on Evers Pharmacy.
- Provide oversight and monitoring to promote organized and efficient medication tracking, refills, and disposal of medication, as needed.
- Oversee compliance with rules for documentation of medication administration.
- Identify when a person's response to a medication or change in condition requires medical review and possible intervention. Notify medical providers of this observation.
- Consult with Medical Director/Provider if a self-directed individual appears to no longer be capable of taking or understanding their medication.
- Collaborate and coordinate with 819 provider staff and provide reports as requested.
- Participate in client clinical review meetings and coordination of care meetings as requested.
- Monitor and document clients' vital signs and additional vitals as needed to ensure client safety i.e., sugar levels, blood oxygen levels, etc.
- Promptly investigate medication errors based on severity index/potential severity to improve staff performance and patient care. Work with Medical Director to identify and when required, report errors to the proper authorities.
- Participate in meetings with staff and vendors that work to improve policies and practices.
- Serve as a resource person by consulting with agency staff.
- Performs other tasks as assigned by Supervisor/s.
EDUCATION AND REQUIREMENTS:
- Registered Nurse from an accredited Nursing school with appropriate educational degrees.
- A minimum of one (1) year of experience as an LPN.
- Must possess an active and unrestricted license as a Licensed Practical Nurse (LPN) in New York State.
- Must possess current Basic Life Support (BLS) certification.
- Must possess current NYS-approved Infection Control certification (4.0 hours, valid for 4 years).
- Knowledge of substance/alcohol dependence/use population.
- Must have specific training in chemical use, abuse, and dependence specific to the services provided.
- Obtain within six months of hire:
- Supporting Recovery with Medications Addiction Treatment (MAT)
- CASAC Canon of Ethics (6 hours)
- Confidentiality related to 42CFR (3 hours)
- Mandated Reporter (2 hours)
- All procedures and functions shall be conducted in accordance with NYS OASAS Local Services Bulletin No. 2012-04: Medication Administration Policies for Administration of Medications in OASAS Intensive Residential Programs issued December 14, 2012, and newer as updated by OASAS.
