We are hiring a Hospice Clinical Supervisor to join the Athena Home Health & Hospice team!
This is an on-the-road position.

As the Clinical Supervisor, you are responsible for ensuring that patient care is coordinated and managed appropriately and ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.
Experience & Education:
- Must hold an active and unencumbered Registered Nurse (RN) license in this state.
- Graduate of a nursing program, BSN preferred.
- Previous experience in hospice setting with two (2) years of management or supervisor experience.
- CPR certification.
Duties & Responsibilities:
- Receives case referrals and reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed.
- Reviews and evaluates each case through home visits, conferences, record review, and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services.
- Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
- Assists clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing a plan of care.
- Attends case conference meetings with hospice personnel to facilitate coordination of care.
- Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and hospice personnel.
- Assists in the screening and interviewing process of new employees and makes recommendations for employment.
- Assists Clinical Director and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs.
- Complies with accepted professional standards and principles.
- Assesses health needs of the community and assists in the development of plans/programs to meet needs.
- Participates actively in quality assessment performance improvement teams and activities.
- Performs other duties and activities as delegated by the Clinical Director.
