We are a virtual-first primary care practice dedicated to improving the primary care experience for seniors as well as our providers. We founded Heyday Health because we saw the health system leaving our elderly loved ones behind and were driven to create a better solution. Our practice is being built from the ground up with the mission of transforming how, where, and when we deliver care to achieve exceptional outcomes.
We are looking for a Health Ally to join our team of patient advocates and providers. The role will include:
- Building close and long-lasting relationships with our patients, their family members, caregivers, and communities.
- Leading patient intake, enrollment, and orientation to the Heyday Health model.
- Providing at-home visits for patient monitoring, education, and support (estimated 30-40% of role will be spent conducting in-home appointments).
- Coordinating a best-in-class patient experience, including scheduling appointments, managing prescriptions, completing follow up, and working with their care team and specialists.
- Supporting the clinical team in the creation of a comprehensive and accurate patient record.
- Working closely with the Heyday Health team of nurse practitioners and physicians to advocate for every patient and ensure the highest quality care.

- Fantastic communication skills and a passion for building relationships with patients and teams.
- Enthusiasm for contributing to and developing innovative models for patient care.
- Commitment to your community and the needs of local seniors.
- Valid driver’s license and reliable personal vehicle that can be used for at-home patient visits.
- Comfort with using or learning how to use electronic health records and other computer systems.
- Dedication to doing the right thing for patients and your team to provide great care.
- Valid US work authorization.
- High school diploma or equivalent.
