Salary Range: [$40.00 to $45.00]

If yes, how you will contribute to the team:
As a Case Manager LVN/LPN, you will oversee and help manage the delivery of care from pre-admission to post-admission. You will help ensure residents and their families receive appropriate utilization of services and coordination of care.
Must have:
- Successfully graduated from an approved and certified LVN program
- Current and unencumbered license to practice as an LVN in the state
- Ability to make quick judgements and work independently
- Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
Nice to have:
- 2 years’ experience as an LVN/LPN
- Active patient or bedside care within the past three years
