Manchester(Only QUALIFIED Healthcare Professionals accepted) Palliative Care-NP - Responsible for the assessment and treatment of palliative care inpatients. Provides patient education and coordinates treatment with Palliative Care Team and agencies for the delivery of consultative palliative care services. Elicit comprehensive history and data about patient’s physical and developmental status and behavior.
Completes physical and psychological assessments based on interview, examination, and available data. Identifies medical and health issues and needs based upon the evaluation.
Formulates differential diagnosis based on history, physical and clinical findings. Reviews clinical data from lab results, x-rays, scans, and pathology reports.
Develop a treatment plan which may include diagnostic testing, pharmaceutical and non-pharmaceutical interventions as well as education. Coordinate treatment and review with collaborating Medical Director, Patient’s attending physician and the Palliative Care Team.
Documents data and encounters with the patient and family in the Electronic Medical Record (EMR). Follows progress of patient care to determine the effectiveness of the treatment plan. Updates and records changes in health status. Modifies treatment to achieve clinical goals. Writes orders for prescriptions, nurse dispensed medications, diagnostic testing and consults as appropriate.
