We have an opportunity for a Lead General Practice Nurse:

The Nurse led model enables team members to develop the full range of Primary Care skills and expertise required to flourish within this growing and challenging area.
As part of Sandwell and West Birmingham NHS Trust, you will have accountability for delivery of services in line with primary care contracts such as QOF, PCCF, and the IIF.
Working with the PCN head of nursing and alongside other Lead nurses, the role requires a committed and strategic thinker to provide leadership support and management to the nursing team.
Main duties of the job
- Ensure the safe, effective, and smooth running of the designated outlet in terms of line management of the nursing team.
- Deliver high-quality long-term disease management.
- Oversee the training and development of staff within the team.
- Be responsible for infection control within the outlet.
- Contribute towards the PCN nursing strategy and delivery of services.
- Work as lead in a specific chronic disease ensuring the highest quality service and care to our population.
- Visit our housebound population and travel between sites; a full driving license and access to a car for work is a requirement.
Person specification
Experience
- Evidence of relevant and recent primary care nursing experience.
- Experience of nurse-led management of chronic conditions.
- Experience of implementing protocols and clinical guidelines.
- Experience of quality initiatives such as clinical benchmarking.
- Experience of data entry.
- Experience of coordinating a Practice Department.
- Experience of Practice Systems clinical systems.
Qualifications
- Primary care nursing specialist qualification or equivalent.
- RGN and Current NMC Registration.
- Clinical supervision: experience of delivery.
- Independent and/or supplementary nurse.
- Post Graduate/Degree level qualification in a relevant Long term Condition or health-related subject.
Knowledge
- Able to demonstrate understanding of the size and structure of working for a larger scale PCN.
- Able to identify how to manage conflict in a team.
- Is able to demonstrate the significance of primary care contracting and its impact on care delivery.
- Displays openness to cross-site working.
- Is an open and confident communicator.
