We are currently looking for a Wellness Director RN to support our community AND our sister community, Markham House.
General Summary: Responsible for the overall coordination of care for all assigned residents. Supervises and mentors the Resident Care Coordinators. Supervises Medication Aides in the delivery of resident care, documentation, and reporting.
Essential Functions/Major Responsibilities include the following:
- Supervises, plans, coordinates and directs the work of the Medication Aides.
- Implement quality improvement programs designed to help deliver the highest possible levels of service to residents.
- Mentor and support the Resident Care Coordinators.
- Mentor and support the Medication Aides in the delivery of medications to residents.
- To sufficiently provide consultation on Residents’ medical needs.
- To adequately respond to emergencies as needed.
- To perform initial evaluations and changes of conditions of Residents as needed.
- Perform periodic monitoring of Resident’s health and wellness in coordination with assessments.
- To delegate special tasks of nursing to medication aides when appropriate with follow-up & evaluation.
- Coordination of provision of health services with outside providers, i.e. hospice, physician, home health agencies, physical/occupational therapy, mental health/discharge planner.
- Oversee and review medication & medication management system, including: upon move-in, med errors, drug regimens, med changes and orders.
- Review and update doctor orders (90 days).
- Assist as needed with Residents’ service plans.
- Other tasks assigned by the Administrator.
Education and/or Experience:
- Must possess at a minimum, a Nursing Degree from an accredited college or university.
- Must possess a current, unencumbered, active license to practice as a Registered Nurse in Oregon.
- Continuing Education in management and industry field.
- High school diploma or equivalent.
- At least three (3) years previous supervisory experience preferred in a hospital, nursing care facility or other related health care facility.

- Demonstrate willingness to help other staff whenever needed.
- Observe standard precaution procedures and all safe workplace measures.
- Report any suspected or observed abuse, neglect or misappropriation of property or funds of a resident to the appropriate licensing agencies.
- Operate department within budget.

Pay: $120,000.00 - $130,000.00 per year
