The Mary Campbell Center is first, last, and always home to 68 residents. The Nursing Supervisor is responsible for supervision of health care staff and delivery of care in support of The Center's goal of achieving a truly homelike environment in which MCC residents live with respect, personal dignity, and independence to the fullness of their abilities. The Nurse Supervisor is charged with championing The Mary Campbell Center's Guiding Principles and mission in a manner that ensures compliance with State and Federal regulations while meeting the specialized needs of residents. The Supervisor must be a positive role model who works effectively, efficiently, and with respect for residents, staff, families, volunteers, professional consultants, and the physical environment of The Center.
MINIMUM REQUIREMENTS:
The Nurse Supervisor at the Mary Campbell Center must: 1) be a good person of high character who demonstrates compassion and understanding, consistently works with a positive attitude and advocates effectively on behalf of residents who have varying degrees of disabilities, and 2) be a graduate of an accredited school of nursing with a current R.N. license in the State of Delaware, BSN preferred; have three to five years clinical nursing experience in an acute, long-term care, or rehabilitation experience; and two to five years of successful, documented supervisory experience. The Nurse Supervisor must also have documentable effective leadership ability; excellent verbal and written communication skills; be CPR certified, have demonstrated assessment, problem solving and organization skills; and demonstrate proficiency in utilizing technology tools to manage resident health reporting and achieve individual, department and mission goals. Knowledge of disabilities and the impact on ADLs is preferred; Wound care experience also preferred.
ESSENTIAL FUNCTIONS:
- Ensures the healthcare staffing resources are utilized effectively in meeting the needs of residents. Manages call-outs, assigns staff or self where necessary to meet situational needs.
- Conducts rounds of the facility at the beginning of each shift and communicates with staff, particularly Neighborhood Nurses regarding assessing/managing residents' care, addressing resident/family concerns, staffing issues, or to communicate need-to-know information.
- Serves as a healthcare liaison for visits by MCC physicians, assisting the physicians during routine weekly resident rounds, as needed. Maintains physician's resident visit calendar for Physician's Orders, History & Physical, and progress review periods.
- Plans, implements, and evaluates the health care provided to residents in accordance with the Nurse Practice Act for the State of Delaware.
- Collaborates with Interdisciplinary Team to ensure healthcare related active treatment goals and objectives are implemented and evaluated according to the Individual Program Plans (IPP).
- Coordinates the assignment and ensures completion of required assessments including the quarterly and comprehensive functional assessment, and documentation related to each resident's Individual Program Plan (IPP).
- Coordinates resident re-admission and ensures completion of medication reconciliation, readmission assessment, Care Plan updates, and Resident Care Activities of Daily Living.
- Is present during mealtimes to ensure mealtime guidelines and resident care plans are being followed.
- Ensures resident's health care plans are current and accurate and reviewed/revised at least quarterly, and when significant changes in condition/new orders occur.
- Ensures that the required documentation is maintained and accurate for neighborhoods and is recorded in the electronic health record and on other required reports and flow sheets according to Center policies and procedures.
- Establishes and maintains effective communication with DON, ADON, residents, families, personnel, therapeutic consultants, and physicians of the Center.
- Serves as a resource staff in the presence of residents or staff behavior or performance issues that require immediate intervention.
- Ensures the proper supervision of direct care personnel through observation and coaching of staff nurses. Observes care being provided by nurses and RCT's and assesses the quality thereof. Observes the activities of staff, ensuring that they are on task unless on a scheduled break or mealtime. Gives direction as necessary and acknowledges good work. Coordinates with Staff Educator for any training needs of healthcare staff.
- Serves as an authorized staff member during the shift to respond to questions, complaints, family questions/concerns or State Agency inquiries.
- Provides documentation and follow-up with residents and families as needed.
- Ensures prompt follow up with labs, consults and radiology reports.
- Ensure prompt follow up with incident management.
- When necessary, contacts the Administrator on Call and/or Manager-On-Duty for management/administrative issues; Maintenance-On-Call for building issues or problems, Technology-on-Call for technology issues and the Transportation Coordinator for transportation needs.
- Assists in the orientation of new staff as directed by the DON or ADON. Provides feedback to the DON and ADON regarding performance appraisal for health care staff on assigned neighborhoods as delegated by the DON and ADON annually.
- Responsible for reporting allegations of abuse, neglect and exploitation to the Division of Health Care Quality as per procedures.
- Assist in managing the MCC Wound Care Program. Provides direct assessment and treatment weekly for affected residents. Provides oversight and direction regarding wound care to neighborhood nurses. Maintains documentation for wound care in electronic charting program and resident chart as applicable. Serves as liaison with physicians and outside treatment entities.
- Adheres to State and Federal licensing and nursing regulations in accordance with RN scope of practice.
- Completes all mandatory in-services and attends staff meetings as required.
- Performs all other duties as assigned.
