Specialty RN II - Wound Clinic - Full Time - Days
The Registered Nurse provides professional nursing care to patients, families, and caregivers in the Medical Group. The Registered Nurse promotes, assists, and advocates for the patient and families' health by completing the nursing process, in support of the patient-centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians, providers, and multidisciplinary care team members. A Registered Nurse is responsible for assessing, planning, promoting, and evaluating quality patient care. Nursing shall perform in accordance with the standards outlined for Registered Nurses in the NYS Education Law, NYS Office of the Professions, by the State Nurse Practice Act, Code of Ethics for nurses, and ANA Standards of Care. A Registered Nurse provides teaching and health promotion to patients and families. A Registered Nurse will follow the mission, vision, I-CARE values, policies, and procedures of Mohawk Valley Health System.
Core Job Responsibilities
- Identify patient care requirements establishing personal rapport with patients, families, and caregivers and understanding of plan of care throughout the patient's experience.
- Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, families, and caregivers.
- Promote patient independence by establishing patient care goals; teaching patient, family, and caregiver to understand disease process, health promotion and preventative care, medication management, and self-care skills, and provide time to answer questions and evaluate understanding.
- In collaboration with care team members and provider supports and adjusts plan of care as appropriate.
- Assure quality care by adhering to standards measuring health outcomes against patient care goals and standards.
- Follow the MVHS and Medical Group patient-centered care model and standards of care set by the State Board of Nursing, state nursing practice act, and other governing agency regulations.
- Protect patients, families, employees by adhering to infection control policies and protocols, medication administration, and controlled substance regulations.
- Utilize standard precautions and practice at all times; maintain equipment, supplies, and inventory as appropriate; maintain a safe and clean work environment by complying with procedures, rules, and regulations; utilize safety devices at all times.
- Document patient care service by charting in patients' medical records accurately and timely in accordance with licensure, regulatory standards.
- Maintain continuity of care with the nursing team by communicating plan of care and follow through with multidisciplinary team ensuring that patient administration of care for all services provided are organized, planned, and delivered.
- Maintain patient confidence and protect operations by keeping information confidential and maintaining a private environment.
- Maintain and anticipate needed supplies, equipment, and medications verifying receipt utilizing MVHS ordering procedure.
- Maintain professional and technical knowledge by attending educational workshops, mandatory in-services; reviewing professional publications and participating in department committee meetings or work groups.
- Engage in quality improvement initiatives including continuing education, participation in system-wide and department-specific teams, identifying nursing practice areas for quality improvement and safe outcomes.
- Maintain a cooperative relationship among the healthcare team by effectively communicating information; timely responding to requests; building rapport; participating in team problem solving and team building with all care team members.
- Provide service excellence to patients, their families, and caregivers that meets or exceeds patient expectations through offering empathetic and compassionate patient-centered care.
- Professionally manage patients', families', and caregivers' needs and concerns using positive service recovery methods; respond to patient requests and make active referrals to subject matter expert/decision maker who can fulfill the patient request; provide culturally sensitive care.
Education/Experience Requirements
Required: Graduate of an accredited associate degree
Preferred: Bachelor of Science in Nursing from accredited nursing program or higher degree
Licensure/Certification Requirements
Required: Current New York State licensure as a Registered Nurse
Preferred: Basic Life Support Certification - Must receive BLS certification within 6 weeks of hire date
