Do you want to be a part of a team who are proud to make a difference to patients and their families?
If the answer to any of the above is yes or possibly, then we would love to hear from you.
The Hospice at Home Team is part of the Community Specialist Palliative Care Team across Northamptonshire. It is a county-wide service providing holistic nursing care for end-of-life patients in their own home, over a 24-hour period. We provide nursing care to patients who require symptom management and end-of-life care at home. Excellent communication and interpersonal skills are essential, as you will be required to communicate sensitively and compassionately with patients who are approaching the end of their life and their families.
Experience of working within the community setting and palliative nursing would be ideal, and a passion to provide outstanding holistic nursing care is essential.
This 24-hour service requires you to be flexible to a varying shift pattern, including early, late, and night shifts, as well as rotational weekend and bank holiday working. This post requires travel across Northamptonshire to patients' homes as needed.
Main duties of the job
The post holder will play an important part in providing holistic care and meeting the specific needs of patients, their families, and those important to them without direct supervision. This will include:
- Assessment of needs and symptoms and implementation of individualized care. Giving appropriate information to their families and involving them in developing the plan of care and its ongoing evaluation.
- Administer prescribed medication as required.
- Ensure accurate records are maintained.
- Ensure a coordinated team approach by using effective communication within the team and other community services involved.
- Providing sensitive information and support to those closest to the patient.
We are looking for a nurse who has personal resilience and can manage the care required for patients and their loved ones who are experiencing emotions expected of those in their last weeks of life.
The post holder will be expected to assist in the learning and development of students and clinicians.
