The Community Health Worker will support the pediatric patients of the SHARK Clinic by assisting their families with connection to referrals and resources.
ESSENTIAL FUNCTIONS
The essential functions include but are not limited to the following:
- Help patients and families reach needed support for children with behavioral health challenges. This may include regular calls to families, managing referrals, and connecting families with resources found in online databases.
- Provide regular calls to adolescent patients with depression and other mood problems to ensure they are following through with referrals.
- Assist families with completing forms and applying for services.
- Advocate for families to access supportive services. Identify family needs and provide available resources or linkages.
- Participate in multi-disciplinary team conferences to discuss clients.
MINIMUM QUALIFICATIONS
Experience
Six months of experience working with the public or with community groups performing duties such as interacting with clients or patients concerning health or social service matters, answering questions, and providing information to clients or patients.
Education
High School Diploma or GED
Certificates/Licenses/Clearances
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation to carry out job-related essential functions.
- Successful clearance through Los Angeles County’s Live Scan process.
- Successful clearance through the Medical Clearance process.
DESIRABLE QUALIFICATIONS
- Lived experience and/or work experience with under-resourced populations in L.A. County or another urban under-resourced area in the US.
- Spanish speaking.
