Registered Nurse (RN)- Case Manager Hospice Spotlight

For a limited time, MaineHealth is offering a $10,000 Sign on bonus for all eligible Registered Nurses with 0-2 years of RN experience and $20,000 for Registered Nurses with greater than 2 years of RN work experience!
- Eligible candidates are hired (offer accepted) into a Full or Part time RN position. Bonus amount prorated for Part time hires; per diem hires are ineligible.
- Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 6 months separation from employment.

CHANS Home Health & Hospice is a full-service non-profit, Joint Commission Accredited agency providing skilled home health, hospice, and an array of community-based wellness clinics and support services in Midcoast Maine. Position Summary
- The Home Care RN supports CHANS HomeCare’s mission, vision, and values by exhibiting the following behaviors: excellence, competence, collaboration, innovation, respect, patient and family-centered care, commitment to our community, and accountability. In accordance with national standards of nursing practice and within the guidelines and policies and procedures of both CHANS and the Maine State Board of Nursing, the Home Care Nurse provides highly reliable, evidence-based, patient-family centered quality care through assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of the effects of nursing care. Care delivery and coordination of patient and family care begins with the initial contact with the healthcare system and may continue across the continuum. Services may be provided on an outpatient and/or community basis. A Home Health Nurse is responsible for traveling to a patient’s home to administer their services and helping patients maintain their independence. In order to meet the patient’s safety and healthcare needs, the case manager works with the provider, interdisciplinary team, the patient, and family to facilitate a plan of current care and coordinate any required follow-up care to promote a balance of quality and cost-effective care.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Bachelor’s Degree preferred.
- License/Certifications: Active nursing license. Current, valid driver’s license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy. Current certification in Basic Life Support for Healthcare providers.
- Experience: Negotiable.
- Understanding of healthcare reimbursement preferred.
- Proficient computer and communication skills.
