Lāna'i Community Health Center (LCHC), located on the island of Lāna'i, is deeply rooted in a unique history of transitions and cultural diversity. This island has undergone economic and lifestyle shifts driven by changing ownership over the years. These transitions have shaped the island's residents, creating a community of great cultural richness, with a significant population of immigrants.
LCHC serves the entire population of Lāna'i, with a special emphasis on those living at or below 200% of the federal poverty level and the underinsured or uninsured. The center provides services in a culturally sensitive manner, offering translation services when needed, and ensures that no one is denied care due to an inability to pay.
As a Federally Qualified Health Center, LCHC accepts all insurances, helping patients navigate their coverage. Role Overview
The Clinical Pharmacist will be pivotal in providing pharmaceutical care services to our community. This role necessitates proficiency in the 340B Drug Pricing Program, management of Pharmacy Benefits Manager(s) (PBM), revenue optimization, vendor and insurance regulatory compliance, as well as experience in retail pharmacy. Collaborating closely with healthcare providers, community health workers, and administrative staff, the Clinical Pharmacist will enhance patient care and streamline workflow efficiencies. Moreover, this position requires a special aptitude for engaging with and serving our small island population, ensuring that LCHC's pharmacy services are fully utilized.
Requirements
- Comprehensive knowledge and application of medication therapy management, patient counseling, chronic disease management, and drug selection.
- Strong understanding of regulatory requirements and compliance related to vendors, insurance, and pharmacy operations.
- Ability to collaborate effectively with healthcare providers, community health workers, and administrative staff to deliver integrated patient care.
- Experience in developing, implementing, and refining workflows to enhance pharmacy service efficiency and effectiveness.
- Excellent communication skills with the ability to educate and train healthcare staff and patients on medication management and health topics.
- Demonstrated ability to foster relationships within the community to expand the utilization of pharmacy services, particularly in a small island population context.
- Proficiency in quality improvement activities including developing and monitoring Performance Improvement Plans (PIPs).
- Experience in preparing and participating in audits related to pharmacy operations and 340B compliance.
- Ability to prepare reports and participate in leadership meetings, including those with enterprise-wide impact.
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy.
- Current and unrestricted pharmacist license in the State of Hawaiʻi; ability to maintain all necessary licenses and certifications for the pharmacy, and thereby all dependent licensure(s) for the facility.
- Minimum of 3 years of clinical pharmacy experience in an outpatient and/or retail setting.

Salary
- Range from $110,000 - $135,000
