Experience & Requirements
- FT Day: 3-12hr shifts/week 6am-6.30pm, rotating weekends
- FT Night: 3-12hr shifts/week 6pm-6.30am, rotating weekends
- Graduate from a Licensed Practical Nursing program.
- Currently licensed as a Practical Nurse in the state of employment.
- Possesses an active CPR certification.
- Knowledgeable about specific state laws and regulations governing practice.
- Satisfactory completion of initial and annual clinical competencies as assigned.
A background check, security clearance, and drug screening are required for this role.
Responsibilities
Position starts at $25.50/hr
As a Licensed Practical Nurse (LPN) with YesCare, you will:
- Contribute to the assessment of the health status of patients.
- Participate in the development and modification of a patient’s plan of care.
- Perform delegated tasks, activities, and functions consistent with prior educational preparation and within scope of practice.
- Provide direct basic care to assigned multiple clients within the healthcare program.
- Provide safe and effective nursing care and assume responsibility for the care delivered.
- Implement patient education based on established teaching plans.
- Utilize a problem-solving approach for decision making.
- Monitor offsite and onsite scheduling collaboratively.
- Monitor consult processes related to policy and contract.
- Participate in Quality Improvement and Patient Care Committee Meetings.
- Ensure onsite and offsite follow-up care of patients along with telemedicine appointments.
- Collaborate with Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders.
- Perform other nursing duties as assigned.
