JOB SCOPE: This is a direct service position requiring specific knowledge of mental health, and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.
Provision of community-based medication management and administration to 68 adults with serious mental illness. This is a full-time position that is split between 2 ACT teams operating in Brooklyn and Queens. Candidates must be detail oriented and have excellent community and organization skills. Experience with psychiatric nursing and driver's license with a clean driving record preferred. Bilingual Spanish speaking preferred. Must have a high school diploma and have a NYS license.
Salary: $ 58,000-$60,000 annually
Scheduled Shift: Monday-Friday 9AM to 5PM
ESSENTIAL FUNCTIONS:
- Participates in daily morning meeting.
- Assists RN with administering IM's and preparing oral medications for weekly delivery by the clinical team.
- Assists RN with maintaining monthly IM calendar.
- Coordinates with Team prescribers and pharmacy to ensure consumers receive medications and refills.
- Transcribe orders and updates Medication Administration Records.
- Assists with logging in medication deliveries and completing toxicology request forms.
- Assists with completion of annual nursing assessments and daily progress notes.
- Conducts weekly Healthy Living group with consumers.
- Performs other duties, consistent with the goals and objectives of the program, as directed by the Team Leader.
Job Requirements
SKILLS AND KNOWLEDGE:
- Experience understanding various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery.
- Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
- Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
- Ensure progress notes are completed within 24-hours of the delivery of the service and are available to be read in the daily organizational meeting.
EDUCATION AND EXPERIENCE:
- Current New York State LPN License.
- Current CPR & First Aid Certificate
- Experience serving the mentally ill population.
