Healthcare Navigator Supervisor - Supervising Community Health Worker

Join our team as a Supervisor and play a pivotal role in shaping Social Determinants of Health outreach, screening, and care navigation programs that advance the mission of the Ralph Lauren Center for Cancer Prevention. In this dynamic role, you will work cross-departmentally and partner with community clinics and organizations to create impactful programs that change lives.
As the administrative lead for our Multidisciplinary Navigation Forum, you'll be at the heart of our collaborative efforts, organizing meetings, crafting agendas, and driving progress by tracking key action items and metrics. But your impact doesn't stop there.
You'll also manage a caseload of patients, helping them and their families set goals, overcome barriers, and develop personalized care plans that connect them to vital resources and support networks. As a dedicated patient navigator, you'll coordinate appointments, facilitate communication, and ensure that every case is resolved with care and precision. Your expertise will be invaluable as the go-to resource for the navigation team when faced with complex or challenging cases.
We're looking for candidates with deep connections to community clinics and organizations in Los Angeles County, extensive experience working with diverse populations and clinical teams, and a comprehensive understanding of the cancer care continuum. Relevant experience includes employment as a social worker (MSW or LCSW); community health worker; nurse (RN); case manager; outreach worker; program director; and other community health, interventional, psychosocial, and social services related positions. Preferred Qualifications:
- Preferred Education: MSW or RN
- Preferred Experience: 5 years in community health, nursing, or social work, with leadership experience
- Supervises: a team of healthcare navigators
- Preferred Field of Expertise: Social Work, Public Health, Community Health, Nursing

The annual base salary range for this position is $81,471 to $108,876. When extending an offer of employment, the University of Southern California considers factors such as (but not limited to) the scope and responsibilities of the position, the candidate's work experience, education/training, key skills, internal peer equity, federal, state and local laws, contractual stipulations, grant funding, as well as external market and organizational considerations.
Minimum Requirements:
- Minimum Education: Bachelor's degree
- Minimum Experience: 5 years
- Minimum Field of Expertise: Experience with process development and change management. Experience managing staff, providing work direction and oversight. Excellent written and oral communication skills and proven ability to work effectively in a team environment.
