Role: Physical Therapist Location: Modesto, CA or Stockton, CA Schedule: Monday-Friday, 8-4 Pay Range: $ 90 - $ 95
Job Summary:
The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home, working in close coordination with the PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function.
The Physical Therapist collaborates with the Occupational Therapist to efficiently operate Client's rehabilitation department.
Physical Therapist is a member of the Client's management team and facilitates collaborative relationships across Client and with external stakeholders to promote the Client's values, team culture and mission.
Duties and Responsibilities:
Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.
Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.
Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP).
Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.
Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
Follow Client policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
Communicate regularly with Supervisor and team regarding workload and priorities.
Timely completion of all mandated trainings and education.
Timely completion of all mandated occupational health screenings as needed.
Exercises flexibility in performing assignments as business needs evolve.
Other duties as assigned.
Qualifications and Requirements:
Bachelor’s Degree in Physical Therapy plus five years’ of Progressively responsible PT experience.
Master’s Degree in Physical Therapy or Doctorate in Physical Therapy preferred.
Valid California Physical Therapy License, or proof of License eligibility.
The qualifying Physical Therapy Degree earned outside of the US must be evaluated to the US equivalent to a Degree in Physical Therapy.
Valid State Driver’s License with a clean DMV record.
Copy of recent Vehicle Insurance required.
Skills and Experience:
Minimum of two years’ Physical Therapy experience in a clinical setting with a frail or elderly population.
Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.
Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
Two years’ supervisory experience with demonstrated ability to mentor and develop team members.
Ability to process accurate information in a timely manner.
Excellent clinical, leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
Highly motivated, inspirational leader who is able to innovatively engage, support and remove barriers for the team to ensure the team consistently meet established goals.
Ability to work independently with minimal supervision
Demonstrated ability to prioritize in a fast-paced environment.
Experience and competency working with people from diverse backgrounds and cultures.

Medical Clearance:
Required that Team Members must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Additional screening may be required as determined by the Medical Director and Human Resources Director in order to protect participants and the community from communicable disease outbreaks (e.g., COVID-19).
Regulatory Requirements Ongoing:
Must not be on any exclusion or debarment from participation in Federal Health Care Programs at any time and must remain in good standing with government regulators such as the OIG, (Office of the Inspector General), SAM, (System for Award Management), CMS (Centers for Medicare and Medicaid Services), etc.
