Job Summary
Job incumbent provides quality behavioral health ancillary services to youth, adults and families. Adhere to the TCC Ch’eghwtsen’ model of service and guiding principle which requires providing timely and effective service along with the ability to interact with others in a way that inspires trust and demonstrates respect, compassion and empathy.

Representative Duties: Under general supervision of the Graf Rheeneerhaanjii Program Manager, job incumbent will:
- Provide Individual and Family Case Management for patients, in coordination with Directing Clinician.
- Provide outreach services to patients, including routine phone and emergency outreach.
- Network with area professionals to ensure follow-up care and referrals, including transfers of care between Psychiatry, Therapy, Inpatient and Outpatient care, Primary Care clinics and Specialty Care clinics.
- Provide coordination and support for assessments, using DSM (Diagnostic and Statistical Manual of Mental Disorders) and ASAM (American Society of Addiction Medicine) criteria. Coordinate directly with other community agencies on inpatient and outpatient referrals for substance treatment.
- Assist with urgent and routine requests for care, including community and provider requests, gathering pertinent history and records, and assisting with triage, in collaboration with Direction Clinician, Psychiatrists and Medical Providers.
- Provide Prevention, Psychoeducation, and support services to patients regarding Mental Health, Substance Abuse, and Wellness.
- Provide and receive appropriate referrals to and from other agencies and resources, including but not limited to: Chief Andrew Isaac Health Center, Fairbanks Memorial Hospital, Anchorage Native Medical Center, Fairbanks community providers, external laboratories/testing services/imaging centers, inpatient and outpatient services both in and out of state.
- Assist with letters, written evaluations, care summaries and community resource agencies as needed, to provide additional patient and provider support.
- Provide assistance with phone calls and follow up calls.
- Complete documentation in a timely manner in accordance with guidelines established by Tanana Chiefs Conference, Indian Health Services, state and federal agencies and regulatory agencies.

Minimum Qualifications
Minimum Qualifications:
- Bachelor’s Degree from an accredited university in psychology, counseling, human development, or related medical or behavioral health field. Applicants without the educational requirements must have a minimum of four (4) years of experience as a behavioral health case manager or coordinator.
- Must have direct patient care experience, including work with children, adolescents or families.
- One year work experience in primary care or behavioral health, addictions field, community case management or similar, is preferred. Must be familiar with mental health disorders, substance abuse and addiction, recovery and relapse prevention.
- Experience working with Native Alaskans preferred.
- Must pass background check pursuant to the State of Alaska background check and federal Indian Child Protection and Family Violence Prevention Act requirements.
Knowledge, Skills & Abilities:
- Experience and education regarding provision of a variety of medical and behavioral health services to adults, children, and adolescents, to include familiarity with: service provision, DSM diagnosis, severely emotionally disturbed children, chronic mental illness, dual diagnosis and integrated treatment for substance abuse and mental illness, treatment planning, treatment coordination, case management, consultation, routine charting, Medicaid regulations, and professional reports. Familiarity with Fairbanks area, Interior Regional, and statewide health services in mental health, medical treatment and substance treatment.
- Cross-cultural treatment skills and knowledge of Alaska Native cultures and traditions preferred. Familiarity with the culture and needs of individuals of Alaska Native descent.
- Strong computer skills using electronic health records and scheduling systems, and Microsoft Office applications.
- Strong verbal communication, analytical and interpersonal skills required.
- Must have initiative and prioritizing skills.
- Must work as part of a team providing quality care.
- Must have excellent customer service skills and the ability to build rapport with patients, while respecting privacy and confidentiality.
- Must be able to exhibit a good understanding of, or ability to quickly learn, Psychiatric medications, illnesses, treatments and care plans.
- Knowledge and ability to adhere to Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2 confidentiality regulations.
- Must demonstrate ethical commitment to mental wellness, recovery and goals of Psychiatry within Tanana Chiefs Conference Health Services.
Supervision: This position has no supervisory responsibilities.
Physical Demands:
Fairbanks area and village travel will be required to meet clinical need. May be exposed to certain conditions, conditions include but are not limited to: travel by small aircraft, vehicle, lack of running water, poor heat, sleeping on floors, and exposed to adverse weather conditions. Job incumbent must be able to physically travel by air and vehicle. Must be able to push, pull, lift and carry up to 40 pounds in support of equipment and supplies. Must be able to work in a busy, and sometimes stressful, clinic environment.
