Job Description
Our client is a provider of reporting and administration for health assessments, and they are looking for a fully on-site Clinical Case Manager to join their team.
The Clinical Case Manager will be onsite in one of their partnered practices and work closely with patients in a busy clinic setting to complete health assessments.
Pay: $26.00-$28.00/hr

Responsibilities:
- Administer assessments to patients.
- Document patient notes daily and create patient activity calendars.
- Hit daily/monthly benchmarks to achieve goals.
- Maintain open lines of communication with supervisor and staff.
- Maintain a positive, professional demeanor with patients, staff, and providers.
Requirements:
- Minimum 3-5 years of experience working in a fast-paced medical practice.
- Must have CNA, CMA, LPN, or similar certification.
- Strong understanding of HIPAA compliance standards.
- Must show high level of personal integrity and ability to maintain confidential information.
- Well organized and able to thrive in a highly independent working environment.
- Excellent oral and written communication skills.
- Proficient in Microsoft-based software and various web-based software and technology.
