Salary Range: $31,461.00 Annually Onwards
Hours: full-time (37.5 hours per week)
Location: Multiple locations based around Lynchburg, VA 24501
Position Summary
The Case Manager I is an entry-level position responsible for providing case management with individuals and their families to establish appropriate linkages; referrals; coordination of care; and to provide regular follow-up on needed services and supports, under the supervision of a qualified Case Management Supervisor. The Case Manager I will identify and assess client needs and evaluate, coordinate and ensure necessary services are provided.
Required Education and Experience
- Bachelor's degree from an accredited college/university in an applicable human services field (including social work, counseling, behavioral science, psychology, or a related field).
- No professional experience required -- less than one year of human services experience. Prefer some relevant experience.
Related Knowledge, Skills and Abilities
Knowledge of the nature of serious mental illness, developmental disabilities, and substance abuse depending on the population served; Ability to observe and report accurately on the functioning of individuals and families; Ability to engage and sustain ongoing relationships with individuals receiving services; Ability to learn and acquire the knowledge and skills necessary to perform all Case Management services to clients; Ability to handle confidential information appropriately; Must possess time management skills; Ability to effectively use active listening skills; Ability to listen to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times; Ability to work independently, performing position duties under close supervision initially and then general supervision going forward. *Ability to document in an electronic medical record system. Knowledge, understanding and commitment to professional ethics; Ability to make home visits and to travel; Strong verbal and written communication skills; To drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to client at other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites; Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed; Willing and available to work modified schedules (weekends, holiday, etc.) as required. *Ability to pass all required training modules.
