Summary:
Combines the use of digital health tools for remote patient monitoring with in-person visits for acute care and chronic disease management. Collaborates with a multidisciplinary team to ensure patients receive high-quality, continuous care. Plans for, delivers, documents, and evaluates the nursing care of patients in an assigned area in terms of their individual needs, according to provider’s orders and in conformance with approved and recognized nursing standards of care, recognized nursing techniques, procedures, and administrative policy. Directs activities of assigned nursing personnel in such care. Observes, assesses, and reports on the condition of patients. Maintains accurate and timely documentation of care reflecting patient’s condition, treatment, and response to care.
Qualifications:
Required:
- Graduate of an accredited school of professional nursing.
- Current registered nurse licensure in the State of Ohio. Acquire Compact License within 90 days of hire.
- 3 years of professional RN clinical experience.
- Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire.
- A thorough knowledge of nursing theory and practice, including basic knowledge relating to nursing such as biological, physical, psychosocial, and medical sciences and their applications.
- Demonstrated interpersonal skills to interact effectively with patients, coworkers, and other Medical staff in a virtual care setting.
- Broad-based knowledge and clinical expertise in assessing, analyzing, intervening, and evaluating patient care.
- Ability to take clinical on-call time.
- Ability to prioritize and manage multiple tasks and function within an ever-changing environment.
- Ability to concentrate for long periods, work well under stress, and be able to think and react quickly in emergencies.
- Ability to coordinate and implement services for multiple patients with a variety of ages and diagnoses.
- Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.
Preferred:
- Bachelor’s degree in nursing (BSN).
- Previous telemedicine experience.
- Previous inpatient hospital care experience.
