Position Description, Qualifications, and Compensation:
Full-time Assistant Director of Nursing
I. JOB SUMMARY
The ADON is responsible for ensuring that patient care is coordinated and managed appropriately. The ADON is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.
II. DUTIES & RESPONSIBILITIES
- Develop and initiate the plan of care and assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s). Initiate proper preventative and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care.
- Ensure a Clinical Manager/Supervisor is available during all operating hours.
- Assure the development, implementation, and updates of the individualized plan of care, which entails communication with all physicians involved in the plan of care and integration of orders, including medication orders, from all physicians.
- Receive case referrals and accept those for which the agency is capable of providing services.
- Review available patient information related to the case, including home visits, to determine home health care needs. Assign appropriate home care personnel as needed.
- Confer with the Attending Physician regarding any questions about an individual's eligibility for services.
- Review and evaluate each case by reviewing the services provided by clinicians, conducting conferences, record reviews, discussing and verifying impressions, and guiding clinicians to promote effective performance and delivery of quality home care services.
- Review patient's medical diagnosis, procedures, medications, and clinical course.
- Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing the plan of care.
- Attend, run, and document case conference meetings with organization personnel to facilitate coordination of care.
- Participate in quarterly record reviews and communicate findings and recommendations to the Director of Nursing and appropriate organization personnel.
- Assist in the screening and interviewing process of new organization personnel and make recommendations for employment.
- Assist the Director of Nursing in the planning, implementation, and evaluation of in-service and continuing education programs.
- Comply with accepted professional standards and principles.
- Participate in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system.
- Participate actively in quality assessment performance improvement programs.
- Perform other duties and activities as delegated by the Director of Nursing and/or Executive Director.

III. JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Must be a registered nurse, with current licensure to practice in the State.
- Three years of management experience in a home health or related health care organization.
- Demonstrated ability to supervise and direct professional administrative personnel.
- Ability to market and deal tactfully with customers and the community.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.
- Excellent observation, verbal, and written communication skills.
- Knowledge of business management and state and federal governmental regulations.
