Position Summary
The Case Manager (CM) will identify, screen, and assist individuals with accessing culturally affirming social support services. This person will work in collaboration with community partners to conduct compassionate outreach to hard-to-reach BIPOC Youth and Young Adult residents ages 16 - 24 who may be experiencing multiple barriers to care (i.e., housing, food insecurity, transportation, etc.). In the role, the CM will focus on the whole person and administer intake, assessments, strength-based case management, advocacy, and community-based referrals and linkages.
Hourly Salary Range: $21.63-$26.44 plus $2 bilingual differential
Essential Duties and Responsibilities
- Attending outreach events and activities.
- Conducting Client Intake by phone, Zoom, or in person.
- Collecting information about clients through assessments and screenings.
- Making adequate referrals and/or linkages.
- Communicating with external organizations to provide additional information and coordination to ensure the client obtains necessary resources.
- Assisting Clients with obtaining and completing forms.
- Understanding and working with the various support systems available in the community.
- Identifying emergency needs and making appropriate referrals.
- Serving as primary advocate and peer support person.
- Maintaining confidentiality of records relating to clients' records.
- Preparing and maintaining notes in the Unite Us Platform and producing monthly reports.
- Attending weekly/monthly staff meetings and participating in case conferences as needed.
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelor's degree in psychology, counseling, rehab counseling, or closely related field.
- A minimum of two years of related experience OR
- A Master's degree in social work, psychology, counseling, vocational rehabilitation, or a closely related field.
- Ability to understand the characteristics of a wide range of intellectual disabilities.
- Personal and professional experience with mindfulness or meditation work preferred.
- Excellent interpersonal skills and the ability to handle emotionally charged situations.
- Ability to effectively communicate with external community partners and clients verbally and in written correspondence.
- Technical proficiency with Unite Us platform (Training Provided).
