PROGRAM SUMMARY
Housing for Health (HFH) is a program office within Health Services Administration, a division under the Los Angeles County Department of Health Services (DHS). HFH was created to support the Los Angeles County Homeless Initiative recommendations to address homelessness in the County’s eight Service Planning Areas (SPAs).
The County-wide Benefits Entitlement Services Team (CBEST) provides targeted benefit advocacy services to assist individuals who are homeless or at risk of being homeless in obtaining sustainable income through government programs such as Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI).
POSITION OVERVIEW
The Assistant Health Program Coordinator position is assigned to one of the CBEST Teams:
- Outreach Team I: Department of Mental Health and Department of Health Services Team
- Outreach Team II: Homeless Systems
- Outreach Team III: Department of Public Social Services and Benefits Advocacy Services Team
- Outreach Team IV: Age-based
- Outreach Team V: Special Populations
- Clinical Team: Records Retrieval
Outreach Teams are responsible for establishing relationships with referral partners to coordinate client referrals for possible enrollment into the CBEST program, assisting in client navigation through the program, and managing all referrals specific to clients’ applications for benefits.
The position will report to HFH supervisory or management personnel and will provide an array of programmatic, clerical, and administrative services that support the processing of client applications for benefits. The position requires frequent travel across Los Angeles County to engage with service providers and attend various meetings.
EXAMPLES OF DUTIES
- Facilitates relations between the agency and the community.
- Interviews applicants to obtain eligibility data.
- Travels to appointment locations to assist clients.
- Prepares necessary forms for client data entry.
- Communicates with patients and agencies to clarify information.
- Assists clients in completing application forms for benefits.
- Explains provisions of public assistance programs.
- Conducts patient outreach and explains program options.
- Organizes and monitors assigned cases to ensure proper documentation.
- May conduct home visits to verify eligibility.

EDUCATION/EXPERIENCE
A Bachelor’s degree from an accredited college or university -OR- Two years of experience in organizing, planning, or implementing a community health program.
CERTIFICATES/LICENSES/CLEARANCES
- A valid California Class C Driver License.
- Successful clearance of a health clearance and live scan process with the County of Los Angeles.
OTHER SKILLS, KNOWLEDGE, AND ABILITIES
- Proficient in Microsoft Office Suite and Adobe Pro.
