Assists with processes to provide care for patients. Tasks to include other duties as assigned. Communicates with patients and their families, Providers, Nurses, pharmacies, ancillary department staff, and outside agencies. Utilizes clinical leadership in problem-solving.
Full-time Hours and Benefits Package: 8 or 12 hour evening or overnight shifts, every other weekend & rotating holidays.
Minimum Requirements:
- Current LPN or RN licensure.
- Prefer prior experience in long-term care or transitional care setting.
- Minimum of 1-year clinical experience preferred.
- Interpersonal communication skills (both verbal and written).
- Customer service and detail-oriented.
- Ability to multi-task.
