Full-Time LPN - Night Shifts - Hawkins County, TN (37642)
Job Overview:
We are seeking a skilled LPN for a full-time night shift position in Hawkins County, TN (37642). This role requires providing 12-hour shifts several days per week, delivering care to a patient with G-tube, Trach/Vent, and CPT needs, along with routine LPN assessments, medication administration, and ADLs.
Schedule:
- Full-Time: Night shifts, 12 hours per shift, days per week to be determined.
Key Responsibilities:
- Conduct regular LPN assessments and administer medications.
- Provide total care, including ADLs, G-tube feedings, and Trach/Vent management.
- Perform Chest Physiotherapy (CPT) as needed.
- Monitor and document the patient’s condition, ensuring timely communication with the healthcare team.

Qualifications:
- Active LPN license in Tennessee.

Responsibilities
- Perform selected nursing procedures within the scope of LPN practice
- Provide patient and family teaching according to plan outlined by the RN
- Monitor patient's vital signs and make specific measurements and observations
- Monitor and record intake and output. Report significant changes immediately
- Observe, assess and record patient's physical and mental condition and reports any changes
- Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
- Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
- Licensed Practical Nurse / LPN license in good standing
- Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
- Active CPR certification
- Familiarity with electronic health records
- Strong problem solving and critical thinking skills
- Reliable transportation, valid driver's license and current automobile insurance
