The Lead Community Health Worker (CHW) serves as a role model to the team by promoting a positive and professional demeanor towards neighbors, staff members, and the team. The Lead CHW will support daily operations of the CHW team as well as manage a small caseload of neighbors and clients. The CHW lead will support the program manager in ensuring high-quality service for all neighbors and patients served by the CHW program. The Lead CHW serves as a leader, thought partner, and active participant in the development of the CHW program.
Required Qualifications:
- Bachelor's Degree or 4 years equivalent experience
- Within 90 days of hire: 8 hours of continued education as assigned by program leadership (i.e., Degreed Training, Community Training, or other as identified and approved by leadership).
- Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
- 3 - 5 years' experience in a Community Health Worker or Community Health Worker-like role or commensurate lived experience.
- 2 years experience related to community outreach, enrollment, and education.
- 1 year experience supporting teams or leading projects.
