RN Care Manager Job Responsibilities:
- Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults with families and other supports for additional information as needed.
- Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.
RN Care Manager Job Requirements:
- Education: Associates degree in Nursing Required, Bachelors Degree preferred.
- Licensure: Wisconsin Licensed Registered Nurse
- Licensure: Eligibility for long-term care functional screener certification is required
- Licensure: WI Drivers License
- Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity. Preferred 2 years of work in community nursing.
RN Care Manager Job Schedule:
Monday- Friday 8:00 AM-4:30 PM.
