Responsibilities include:
- Chronic care management - communicate monthly through pre-arranged telephone calls with patients enrolled in the chronic care management program.
- Document the current status of the patient (medications, health questionnaires) and encourage patients to self-manage their chronic conditions and make progress towards the goals set by the patient and their physician.
- Obtain and document vital signs including but not limited to: blood pressure, temperature, pulse, respiration, height, weight, health history, current medications, and presenting problems.
- Schedule patients for diagnostic testing and referrals.
- Communicate with patients about test results and plan of care, including prescription renewals and inquiries.
- Perform other related duties as assigned.
- Must be able to think and work independently.
- Experience with EMR preferred.
