- Position: Part-time to Full-time Home Health Registered Nurse (RN)
- Wisconsin Locations: Central WI, Milwaukee, Waukesha, Sheboygan & Manitowoc (and surrounding communities)
- Illinois Locations: Gurnee area
- Competitive Hourly Wage: Up to $45.00/hr.

QUALIFICATIONS:
Registered Nurse with current license in the state of Wisconsin/Illinois; One year of experience as an RN in an acute care setting, preferably two years of experience. Home Care experience is desired.
ESSENTIAL FUNCTIONS:
- Performs assessments and develops individualized patient care plans and manages multi-disciplinary care as applicable.
- Provides skilled nursing care in a patient's home as ordered by the attending physician in conjunction with the plan of care.
- Supervises LPN's and Home Health Aide's in a patient's home.
- Complies with agency policies and procedures. Promotes and maintains an agency environment that is in compliance with federal, state, and local regulations.
- Submits clear and appropriate documentation in a timely manner and documents patient education.
- Communicates effectively to physicians, supervisors, and coworkers in a timely manner.
- Collaborates with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients. Participates in case conferences to promote coordination of care.
- Promotes continuity of care with appropriate admissions, transfers, and discharges.
- Keeps abreast of developments in healthcare, attends in-service training, and agency meetings as required.
- Reports to appointments as scheduled.
- Maintains a professional appearance.
