Position Description:
The overall goal of the Case Manager position consists of rehabilitation, environmental support, guidance, and resources coordination. The Case Manager is essential to assist youth & young adults in gaining access to necessary services and in creating environments that promote resiliency and support the emotional and functional growth and development of the consumer/family.
Role and Responsibilities:
- Assist with youth referred from the Atlanta Public Schools & Fulton County Schools, Department of Juvenile Justice, Probation Office, Atlanta Police Departments, Fulton County Courts, and youth appointed for service by Therapists.
- Display positive role-modeling to members.
- Promote self-esteem and healthy boundaries.
- Promote good hygiene and appropriate personal appearance among young adults.
- Reinforce socially acceptable behaviors (ex: manners, eating habits, personal safety) and teach thoughtful decision-making.
- Teach and reinforce professionalism in work tasks and employment.
- Assess members' history of trauma, strengths, and needs.
- Conduct YASI Assessment.
- Administer CANS (Child & Adolescent Needs & Strengths) and be aware of ACE score and its implications on service planning.
- Develop & monitor Individualized Service Plans.
Coordinating Services and Collaboration:
- Transport members to and from facility and/or community sites.
- Meet with assigned members at a minimum monthly.
- Monitor, schedule, facilitate and/or participate in Team Meetings for members.
- Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to members.
- Collaborate and consult with other providers and entities in the provision of best care services to members.
- Monitor the provision of services provided.
- Provide transportation of youth as needed or required.
Management of Caseload:
- Make necessary partnerships to meet members' health, well-being, education, mental health, and employment needs.
- Research and/or attend trainings to educate self on best practices to provide adequate services to members.
- Employ tools to maintain compliance with governing agencies or entities (i.e., DBHDD).
- Manage budget and ensure appropriate spending.
- Oversee member’s progress and program participation.
- Facilitate skill building and develop activities related to life skills, mental health resiliency & substance use recovery.
- Facilitate skill building discussions individually and/or in groups (ex. Seven Challenges, ATRIUM model).
- Facilitate Diversion Groups.
- Provide member updates in Case Notes.
- Complete Incident Reports as needed prior to shift end.
- Monitor completion of deadlines (i.e., Monthly Summary Reports).
- Provide reports to governing/referral agencies regarding participation of member.
Performance and Conduct:
- Maintain confidentiality of member information.
- Maintain appropriate professional boundaries.
- Follow the CHRIS 180 Code of Ethics.
- Follow mandated reporting requirements of GA Code.
- Support and maintain an environment of emotional and physical safety for members, staff, and other stakeholders.
- Demonstrate customer service.
- Promote a positive image of CHRIS 180 programs at all times.
- Work as a contributing and collaborative team member.
- Respond to consumer and peer needs in a timely fashion.
- Answer telephone calls and emails accordingly.
- Respond to requests for information from stakeholders in a timely fashion.
Maintain Personal Information and Training:
- Attend and participate in staff meetings as scheduled.
- Attend and participate in required trainings as scheduled.
- Maintain required certifications.
- Review, understand and be responsible for program policies.
- Provide documents in a timely fashion to Human Resources.
- Report changes in status to HR as required by policy.
Qualifications/Competencies:
- Experience working with youth and transition age young adults with mental health and/or substance abuse diagnosis (co-occurring).
- Bachelor’s Degree* in the field of social services (Ex. Psychology, Social Work, Sociology).
- Skilled in assisting and maintaining consumers in their family homes and/or community.
- Ability to effectively relate to consumers, parents/custodians of all social, economic, ethnic, and educational backgrounds, with firmness, fairness, and concern.
- Solid decision-making ability related to the safety and well-being of consumers, families, community, and staff.
- Dependability, emotional stability, and physical capability.
- This position requires some physical functions that are essential to satisfactory performance of the outlined job responsibilities, i.e., sitting, standing, walking, driving, speaking, hearing and the ability to physically restrain (when necessary).
- Ability to recognize when assistance and consultation is needed.
- Ability and/or experience in working with severely emotionally disturbed children, adolescents, and their families.
- Must be free to travel as the position demands.
- A valid driver’s license in the State of Georgia.
- Must be at least twenty-one (21) years of age.

Transportation:
As part of this role, there may be a requirement to provide transportation services for clients, ensuring safe and punctual transportation to and from programming. This includes responsibly driving clients while adhering to agency driving standards, maintaining a clean driving record, and prioritizing client safety and comfort during transit.
Schedule:
This position is a full-time job, with work occurring Monday through Friday, and occasional availability on (2) Saturdays per month. Workdays typically consist of 8 hours per day. Scheduled start and end times vary based on school or summer hours to accommodate the needs of the school-age clients. The specific schedule will be determined in coordination with the Program Manager and may include evening hours to accommodate participant availability and program events.
