Description
As the primary coordinator of the Elder at Home care team, the RN-Care Manager provides across-the-continuum, customized care coordination services and management of transitions of care for the most medically fragile PHP Medicare Advantage, at-risk patients and their families/caregivers, incorporating patient-centered interventions and patient engagement strategies to promote self-management in the home/community setting.
We visit patients in their homes with the goal of reducing ED utilization and hospitalization. We do this through scheduling urgent visits and, if needed, provide IV fluids, IV medications, EKG and assessments in the home with our provider group of eight physicians.
This position is per diem, and there are many opportunities for shifts. We are not currently covering weekends and 1-2 holidays/year.
Our geographic area covers Portland Metro (as far East as Gresham/Troutdale), West to Forest Grove, South throughout Yamhill County (Newberg/McMinnville), and southeast including Oregon City and Clackamas. The assignments do not cover the entire geographic area each day but a portion of it. However, that area could be different each day.

Required Qualifications:
- Oregon Registered Nurse License upon hire.
- 2 years Clinical medical-surgical, acute care or long-term care experience that also includes:
- 1 year in community-based/ambulatory care.
- 1 year in care of the frail elderly.
Preferred Qualifications:
- Bachelor's Degree in Nursing.
- Home health, hospice, palliative care, managed care, or skilled nursing facility background.
- 1 year Care management of diverse patient populations.
