The role you’ll contribute:
Responsible for continuity of care across the continuum with emphasis on quality patient outcomes and cost-effectiveness through decreased length of stay, and control of resource utilization. Provides patient/family instruction, scheduled physician appointments, and any additional discharge collaboration/coordination in the hospital's EMR and coordinates activities according to patient's need.
The value you’ll bring to the team:
- Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self-care, nursing facility, home health, physical / psych facilities
- Acts as liaison with physician to assure appropriate and complete documentation
- Prioritizes activities and completes work assignments on a timely basis while functioning with minimal supervision
- Assists physicians by providing information, suggestions, alternatives, and timely referrals for discharge planning
- Provides continuity of care by coordinating the flow of information between hospital departments and outside organizations
- Reviews patient medical records for PI / Risk Management per established criteria and provides appropriate documentation of adverse patient occurrences
- Provides information to physicians and the community on the benefits and alternatives home health and support service programs can provide
The expertise and experiences you’ll need to succeed:
- High School Graduate or equivalent
- Licensed Practical/Vocational Nurse within the State of Florida
- Three (3) years LPN health care experience
Preferred qualifications:
- Five (5) years LPN health care experience
- CPR certification
- Home care or skilled nursing facility experience
