Job Opening: Licensed Vocational Nurse (LVN)

Pay Range: $27.00 - $30.00 per hour
Reports To: Clinical Supervisor
Position Overview
We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in the field. In this role, you will follow a physician-approved care plan while collaborating closely with the Case Manager/RN and interdisciplinary team. Key Qualifications
- Active California LVN license from the California Board of Vocational Nursing and Psychiatric Technicians.
- 1+ year of nursing experience (community/home health or medical/surgical preferred).
- CPR certification (current and valid).
- Strong clinical judgment with knowledge of current nursing practices.
- Effective verbal and written communication skills.
- Must pass a pre-employment physical (including Mantoux TB test or chest X-ray).
- Able to walk, bend, stoop, and lift objects as needed.
- Licensed driver with insured vehicle (non-drivers must sign an Auto Release of Liability).
- Fluent in English, with excellent organizational skills and the ability to work independently.
- Familiarity with HIPAA, mandated reporting, and healthcare regulations (e.g., Title 22).
Primary Responsibilities
- Develop and implement nursing care plans in consultation with the interdisciplinary team.
- Provide safe and effective care in accordance with the LVN Practice Act.
- Educate patients, families, and caregivers about care plans, tests, and health conditions.
- Maintain accurate and timely documentation, ensuring compliance with agency policies and third-party requirements.
- Monitor and report any significant changes in the patient’s condition to the physician or RN.
- Participate in home health service coordination, collaborating with the Case Manager or Clinical Supervisor as needed.
- Attend case conferences, staff meetings, and in-service training sessions.
- Follow proper infection control procedures and adhere to Standard Precautions.
- Support the RN and physician with specialized procedures, ensuring aseptic techniques are followed.
- Arrive on time for assigned visits/shifts and report cancellations promptly.
- Maintain up-to-date credentials and stay informed about healthcare regulations.
- Report incidents, fraud, abuse, or patient safety concerns as required.
Physical Requirements
- Frequent walking, bending, stooping, and lifting as required for patient care.
- Use of hands to operate medical equipment and technology.
- Must be free from illness and infections to protect patients and staff.
