Optum is committed to supporting medical practices to transition to sophisticated team-based care models that meet the needs of individual patients and populations of patients. Foundational to this transition is moving the business model from primarily Fee-For-Service (FFS) to FFS plus Value Based Care (VBC). This transition shifts the emphasis from a focus on volume to a focus on quality and resource stewardship.
Value Based Care relies on excellent documentation and coding to accurately capture the burden of illness and demonstrate the quality of care each and every patient in the population. This must be done at least once per calendar year. The Clinical Performance ARNP (also called the “Embedded NP” or ENP) has a critical role in meeting patients where they are and ensuring that all gaps in care and documentation are closed in a timely fashion. This often involves travel to an independent clinic site within the Optum Care Network of contracted practices. Travel time is covered and travel expenses are reimbursed per mile. If overnight travel is required then the schedule will be pre-arranged in collaboration with the ARNP and appropriate accommodations in the schedule will be made on an as needed basis.
The clinicians we seek are those who practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address our patients’ needs. We are looking for clinicians who are comfortable in various settings (including the home), able to establish rapport with patients, and willing to be flexible. This position will not have its own panel of patients, and is not responsible for ongoing therapeutic management of acute or chronic conditions. Sites of care could include (but are not limited to):
- Primary care clinics (particularly within practices affiliated with OptumCare Network
- Various specialty clinics (exact locations in scope will be determined depending on clinician home base)
- Home visits, long-term care facilities
- Virtual visits
Primary Responsibilities:
- Address quality care gaps
- Identify and address factors related to chronic conditions that may not have been recognized and to further work with patients to better achieve optimum health status
- Perform and document appropriate screening tests for chronic conditions or their complications
- Educate patients about resources available to them within OptumCare Network
- When necessary, link patients to care management or other services

Required Qualifications:
- Advanced degree as a Nurse Practitioner (ARNP or DNP)
- Current and unrestricted license to practice medicine in Oregon State
- Current and unrestricted DEA certificate
- Current license to operate a motor vehicle and a reliable vehicle
Preferred Qualifications:
- Experience as a primary care clinician
- Experience in chart review and documentation inside an Electronic Medical Record (EMR)
- Experience with geriatric patients in value-based care contracts such as Medicare Advantage
Essential Technical/Motor Skills:
- Excellent range of knowledge with respect to the practice of medicine. Ability to speak clearly and communicate with patients, families, agencies, other providers and staff regarding patient care
- Competency in the range of Microsoft Office applications and similar tools, including but not limited to Word, Excel, and PowerPoint
- Ability to communicate through a language translator (both live in-person and remote telephonic translators)
Interpersonal Skills:
- Ability to develop positive interaction with patients, families, physicians, administrators and co-workers in order to effectively care for the patient
- Key competencies include: curiosity, communication, collaboration, relationship-management, storytelling, and adaptation to change
