Job Description
As an RN Care Manager, you will be at the forefront of providing clinical care management services. This role requires flexibility and a commitment to enhancing the well-being of our clients through regular assessments and care coordination. This position is perfect for those with a passion for geriatric and home health care.
What a Typical Day Will Look Like:
- Conduct thorough assessments of clients’ conditions and monitor their well-being through regular visits (at least monthly).
- Collaborate with Geriatric Care Managers (GCM) to coordinate care and health-related services effectively.
- Complete Patient Review Instruments (PRI) and screens as needed.
- Manage medication regimens for clients, including pre-pouring medications and picking up prescriptions from pharmacies.
- Record vital signs and document each client visit meticulously.
Position Goals:
- Your primary mission is to provide high-quality clinical care management to improve our clients’ health and quality of life.
- Utilize clinical expertise and compassionate communication to support clients and their families.
- Ensure accurate and comprehensive documentation of care and services provided.
Qualifications:
- 5+ years of experience as a Care Manager or in a similar role preferred.
- Active RN license with a clean driving record and access to a vehicle.
- Strong clinical skills with experience in geriatrics and home health specialties.
