Wound Care Nurses with Outpatient Wound Care Nurse – Case Management experience are invited to interview for an opening position to work in our St. Petersburg, Florida office. HealPrecisely is a highly innovative and forward-looking mobile wound care program dedicated to promoting the healing of chronic wounds and decreasing the number of surgical amputations that are often performed as a last resort.

SUMMARY OF POSITION:
Full-Time Position – Wound Care Nurse – Case Manager for our Advanced Wound Care Mobile Company located in St. Petersburg, Florida. This position may include some travel in surrounding areas with our providers.
Position Description: The Wound Care Nurse – Case Manager works with HealPrecisely’s Clinical and Operations Team to provide wound care to patients in their homes, both in the community and independent living, as well as long-term care rehab facilities. This position is responsible for providing wound care management in collaboration with the Clinical Manager, Medical Director, and operations team.
Training Provided & Supervision Received: Clinical Manager & Educator, Medical Director; Physician Steering Committee; Wound Care Experts & Medical Oversight and Support Department.
Supervision Exercised: Clinic-based & Mobile care team.
Essential Functions/Responsibilities
- Per Orders – provide direct patient care and case management services to patients with wounds.
- Assist providers with comprehensive evaluations and assessments to determine patient needs, focusing on aggressive treatment plans and support in their own homes.
- Work collaboratively with interdisciplinary healthcare team members, including providers, nurses, care coordinators, and other healthcare professionals to coordinate and implement patient care plans.
- Monitor patient progress and provide ongoing case management.
- Engage in effective wound care planning and ensure smooth transitions of care for patients.
- Apply case management principles to optimize patient outcomes and efficient resource utilization.
- Educate and support patients and their families regarding healthcare needs, chronic disease management, and lifestyle choices.
- Keep daily inventory of skin substitutes, allocating for weekly placement.
- Assist Clinical Manager in tracking wound center statistics.
Applicants must be able to self-direct and motivate during assigned hours of service and must be able to drive to sites of service if needed (Financial stipends provided).
Education/Qualifications:
- Must possess a current Florida RN license.
- At least 2 years of nursing experience, with a preference for experience in geriatric care, hospice, home health, and wound care.
- Strong understanding of wound physiology, chronic disease processes, and treatment modalities.
- Excellent communication and interpersonal skills for effective collaboration with patients, families, and a diverse healthcare team.
- Demonstrated ability to manage multiple cases effectively while maintaining meticulous attention to detail and compassion.
- BLS and CPR certified.
- Ability to react effectively in emergent situations.
- Ability to communicate clearly and establish/maintain working relationships with patients, providers, and referral sources.
- Ability to work independently with interdisciplinary collaboration.
- Completes required number of continuing medical education hours for license and certification.
- Attends required meetings, as requested. Ability to drive from home to patient locations, if/when needed.

Physical/Mental Demands:
- Requires full range of body motion to provide the care needed to patients in their environment.
- Requires visual and hearing acuity.
- Ability to lift a minimum of 50 pounds and provide excellence in care despite varying community conditions.
Essential Functions:
- Works closely with our Providers to ensure we are meeting the needs of our patients in our community for Advanced Wound Care Mobile Services.
- Ongoing collaboration with the Clinical & Operational Management Team.
- Knowledge of up-to-date wound care products and therapies, including application and management of these products.
- Other duties as assigned.
Assist Providers with (not limited to):
- Development and managing individualized wound care treatment plans for optimal healing outcomes.
- Orders, reviews, interprets, and integrates diagnostic studies and laboratory findings for initiating or modifying the treatment plan.
- Prescriptions and/or durable medical equipment to assist in healing wounds.
- Provides advanced wound care treatments per order as needed.
- Reviews patient’s prior medical records and maintains their current medical record related to the wound diagnosis and contributing co-morbidities.
- Identification of wound care quality measures and provide data and documentation for tracking of quality measures.
- Instructs patient and family regarding wound treatments and management of plan for optimal wound outcomes.
