As part of the VNA Health Group, our team of dedicated clinicians continues to grow, making house calls to private residences, senior housing, and assisted living facilities throughout Ocean, Monmouth, Middlesex, and beyond.
We are seeking a passionate Nurse Practitioner to join our team, delivering primary care to homebound patients in Ocean County. In this role, you’ll provide high-quality care, assess and manage symptoms, and promote the highest possible quality of life for patients and their families—right in the comfort of their homes.

Responsibilities
- Deliver high-quality patient care by assessing, diagnosing, and developing comprehensive care plans.
- Monitor patient conditions and adapt treatment plans as necessary.
- Serve as a compassionate liaison between patients, families, and other healthcare providers.
- Participate in the organization’s Quality Assurance and Performance Improvement (QAPI) program.
- Adhere to safety and infection control policies and organizational standards.
Qualifications
- Master’s degree in Nursing from an NLN-accredited program.
- Licensed Advanced Practice Nurse (APN) in NJ with prescriptive authority, CDS, and DEA.
- National certification as an Adult or Geriatric Nurse Practitioner.
- 2+ years of APN experience or extensive RN experience.
- Excellent communication, organizational, and interpersonal skills.
- Strong computer proficiency.
Working Conditions: This role involves working both indoors and outdoors with patients and their families. Valid driver’s license and auto insurance required.
