Director of Case Management
As a Director of Case Management, you play a pivotal role in overseeing the entire case management process. Your responsibilities include ensuring effective patient care, adhering to professional standards, and optimizing resource utilization. Here are the key duties and qualifications:
Duties and Responsibilities:
- Program Development:
- Develop and implement case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation.
- Evaluate patient care data to ensure alignment with clinical guidelines and organizational standards.
- Patient Care Coordination:
- Monitor patient progress, consult with the healthcare team, and adjust treatment plans as needed.
- Expand case management programs to enhance patient recovery and reduce hospital stays.
- Collaboration and Decision-Making:
- Work closely with nurses, physicians, therapists, and other healthcare professionals to create a seamless care experience.
- Make resource allocation decisions and manage funding for case management services.
- Compliance and Staff Management:
- Maintain compliance with federal and state regulations related to case management.
- Participate in hiring, training, and evaluating case management staff.
Qualifications:
- Bachelor’s degree (Master’s preferred).
- RN, LSW, or LCSW license.
- 3+ years of experience in a healthcare case management role.
- Strong leadership skills and familiarity with case management guidelines.
Job Type: Full-time
Pay: $130,000.00 - $140,000.00 per year

Education:
Experience:
- Discharge planning: 3 years (Preferred)
- Hospital Case management: 3 years (Preferred)
License/Certification:
- LSW (Preferred)
- Licensed Clinical Social Worker (Preferred)
- RN (Preferred)
Ability to Relocate:
- Lawton, OK: Relocate before starting work (Required)
