Responsible for the ongoing development, organization, implementation and evaluation of the home care services in accordance with the accepted policies, procedures and practices of Perry County Memorial Hospital and state, federal, and accreditation regulations for home care services.
Job Duties
- Supports and promotes hospital’s mission, vision, values and goals.
- Models PCMH’s Standards of Behavior.
- Participates actively as a member of the hospital’s management team (LEAN, committees/teams, meetings, etc.)
- Models and ensures staff’s compliance with organizational and departmental policies, procedures, and protocols, including confidentiality and privacy.
- Manages all aspects of Home Care Services’ department:
· Interprets and ensures compliance with laws, accreditation standards, licensure rules, standards of care, etc., as applicable to HCS department.
· Develops and implements policies, procedures, plans and protocols to comply with all regulations and rules.
· Manages departments’ personnel functions, in conjunction with HR when applicable, including but not limited to hiring, orientation, coaching, evaluation, scheduling, job descriptions, competencies, employee development, etc.
· Develops, implements and promotes departmental quality plan, processes and measures in conjunction with the hospital’s QI programs and to meet national, state and accreditation standards.
· Responsible for department’s financial management, including FTEs and productivity, as well as expense and capital budgets.
· Ensures patient care is interdisciplinary, ethical, and meets/exceeds quality and safety standards.
· Ensures appropriate and complete documentation by all disciplines.
· Plans, coordinates and documents all departmental meetings/conferences (Clinical Record Review, case conferences, staff meetings, etc.).
· Responsible for communicating organizational and departmental goals and information to staff via monthly staff meetings and other means as necessary.
· Develops and implements a strategic plan for HCS.
· Responsible for departmental marketing in conjunction with Marketing Director.
· Collaborates with community agencies and vendors as appropriate.
· Provides timely reports and communications with Chief Nursing Officer.
- Communicates and works harmoniously and effectively with others at all levels within the organization, including senior leaders, peers, subordinates, patients, visitors and physicians/licensed independent practitioners, as well as external contacts.
- Performs, as needed, general patient/client care duties as outlined in the “Home Care Staff Nurse” job description.
- Responsible for staying abreast of new developments within profession via continuing education methodologies.
- Performs other duties as assigned.
Education
- Graduate from an accredited school of nursing preferably with a Bachelor of Science degree. Due to frequent regulatory/licensure/accreditation changes, continued education through workshops and seminars is required.
Licensure/Certification
- Must be licensed to practice as a Registered Nurse in the State of Indiana.
- Must maintain current CPR certification.
- Must provide copies of licensure/certification for personnel records including renewals.
- Must provide proof of current insurance against liability for personal injury, death or property damage that may arise due to employee’s ownership, maintenance and operation of automobile.
- Must provide proof of valid driver’s license and renewals as mandated.
- Must provide own transportation to and from patient homes
Experience
- Experience in management and home care services preferred.
