As the Clinic Case Manager, RN, you are responsible for the general oversight, assessment, planning, and construction of the nursing care delivered to the PACE participant. You may delegate direct care to nursing assistants on the team but you retain responsibility for ensuring that interventions are carried out in accordance with the participant’s care plan and the orders of the participant’s medical providers.
Primary Responsibilities
- Completes RN assessment during initial, semi-annual, annual and significant change evaluations.
- Assesses participant through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT.
- Ensures participants care plan is reflective of all nursing interventions and participant diagnoses.
- Implements and carries out care plan interventions and testing, treatment, or other provider orders for participant.
- Safely and effectively administers medications, tests and treatments as ordered in the PACE day center or participant home.
- Manages participants medications and refills with appropriate authorization by the PACE provider.
- Implements, directs, evaluates and provides oversight of nursing care and care provided by certified nursing assistants.
- Provides health teaching to participants, families, and caregivers.
- Participates in Quality Improvement activities as requested.
- Provides On-Call coverage to troubleshoot, advise, teach, coordinate and ensure participant care is delivered.
Employment Specifications
Education/Experience:
- Degree in nursing from an accredited college or university.
- At least two years’ as staff nurse, preferred.
- Experience with complex and older populations.
Licensure, registry or certification required:
- Current Nursing License to practice in North Dakota.
- Current CPR/BLS certification.
- Proof of a valid driver’s license, good driving record and auto insurance as required by state law.
