You’re a clinician, a patient advocate, and an educator — and you excel at every one of them.
You will play a key role in the education of patients by providing telephonic support and assisting in coordination of available resources to patients who have been prescribed our customer’s product and have opted into the Nurse Navigator support services. The Nurse Navigator will assist in the coordination of initial Injection training for SQ self-injection and provide follow-up with patients at pre-determined intervals during the therapeutic process. You will:
- Provide virtual support to patients newly prescribed the product.
- Maintain flexibility to vary assignments at short notice to account for regional program overflow, vacations, etc., of Nurse Navigator team members where you may provide telephonic or video injection training, accelerate disease state awareness, and educate staff on Nurse Navigator programs and our customer’s resources.
- Integrate compliantly, per business rules, with Assembia HUB and Patient Care Coordinators, reimbursement specialists, and other matrix partners with the goal of optimizing the patient experience and creating adherence to the drug therapy.
- Master all aspects of the disease state and knowledge of patient resources as it applies to Lipodystrophy.
- Act as liaison to various patient resources pre and post injection and throughout the patient’s participation in the Nurse Navigator program. Follow up via telephone/video support to patients undergoing treatment.
Minimum Requirements:
- Active RN license.
- Prior experience with virtual or home care experience.
- Ability to travel for meetings.
- Must live within territory or within territory boundaries.
Desired Requirements:
- Minimum 2 years industry experience.
- HIV and hepatology experience.
