POSITION SUMMARY:
The LPN provides individualized nursing care and comprehensive services to patients and families in their own homes under the supervision of a Registered Nurse.
PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:

- Provides basic patient care under the supervision of a Registered Nurse in accordance with physician’s orders
- Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action
- Educates patient and families in self-care and health management
- Attends case conference as requested
- Adheres to policies and procedures
- Participates in agency quality management and safety activities

Educational Requirements and Experience:
- Graduate of an accredited school of nursing
- Current license to practice as a practical nurse in the state of Massachusetts
- Current CPR required
- Two years nursing experience
- Current MA driver’s license and reliable transportation
